• Develop network of high-quality PAC partners (e.g. SNF, home health, IPR, LTAC, etc.) and maintain engagement through open communication, regular on-site visits and routinely scheduling meetings
• Identify and collaborate with community agencies to address Social Determinants of Health (SDOH) needs. Routine communication and collaboration with the Director of Population Health
• Real time communication and follow up on identified areas of opportunity or service failures
• Work with PAC partners to promote accountability, quality, and efficient and seamless transitions of care
• Identify opportunities to build upon UCMC's value proposition for PAC partners
• Establish governance structure for PAC network
Data Collection/Quality Assurance:
• "Monitor and report Key Performance Indicators (KPI) for the PAC network, including quality metrics (e.g., CMS quality star ratings, readmissions, length of stay, patient satisfaction) and operational metrics (e.g., response times, acceptance rates, % denials), and promote PAC network partner accountability for quality and operational performance
• Responsible for ongoing maintenance of contracts and provider agreements including, but not limited to, amendments and addendums
• Responsible for evaluating and maintaining care coordination technology contracts
• Analyze PAC network performance data to identify trends and opportunities
Internal Collaboration and Education:
• Participate in defining business strategies, site challenges and critical problems to address
• Collaborate with UC physicians and service lines to assist with preop discharge planning
• Identify and communicate service specific clinical protocols for PAC
• Maintain open communication with case management and social work teams across they system to address barriers with PAC placement and home care services
• Develop internal case management/social work and patient-facing materials related to PAC network
• Attend staff meetings, huddles, outlier meetings
