Annie Ross Housing Services Case Manager (Manufacturing)
Northwest Housing Alternatives is offering an exciting opportunity to assist in securing and stabilizing housing for vulnerable families experiencing homelessness in Clackamas County.
Responsible for providing case management and support services from a client-centered, trauma informed perspective to individuals and families at risk of and who are currently experiencing homelessness. Case Managers work with each household on housing navigation, placement, and a wide array of housing stabilization activities. NHAs HomeBase services include homelessness prevention, diversion, and rapid re-housing. Program participants include all shelter participants from the Annie Ross House Shelter and various community based Permanent Supportive Housing units that NHA supports.
This is a full-time Monday through Friday 8:30am to 5pm (plus an occasional evening and weekend day). Starting pay is $59,363 - $74,546 per year depending on relevant experience. This position participates in the overnight on-call rotation system providing as needed emergency phone support for households within shelter. The overnight on-call rotation has an additional $30 nightly stipend plus pay for any actual time worked.
Core Duties and Responsibilities:
Conduct an in-depth eligibility assessment and build supportive rapport with all program participants upon intake. Engage participants in exploring strengths and identifying barriers to housing or stability within the community in the event of homelessness prevention. Cooperatively develop an individualized housing stabilization plan according to the unique household situation. Schedule regular meetings with participants to monitor progress toward housing stabilization. Provide information and referral of community resources to participants to achieve goals. Assist in connecting with those resources as needed. Provide advocacy as appropriate. Landlord outreach and engagement with the goal of being able to house new participants and prevent evictions for existing participants. Provide skills training in navigating the social service system, daily living activities , and finding and applying for affordable housing to empower participants. Assist participants in locating and obtaining safe, affordable housing. Facilitate the appeal process when participants are denied housing. Determine appropriate levels of financial assistance and coordinate the provision of housing assistance directly with vendors such as landlords or utility companies, etc. Facilitate move-in activities with participants, including conducting housing inspections, coordinating with landlords, and assisting in executing leases. Ensure units abide to Lead Base Paint Safety Standard laws. Coordinate household-specific activities with Shelter Engagement Specialists. Become knowledgeable about the various funding regulations and eligible expenses per grant. Complete all participant documentation according to HMIS Standards to facilitate accurate data entry into the HMIS system. Complete accurate and timely documentation of intake, assessment, case plan, participation agreements, financial assistance requests, lease documents, case notes, and any other paperwork required by the program. Ensure the case file is complete and accurate. Provide transportation to program participants as appropriate. Attend staff meetings, workshops, and training as scheduled. Meet regularly with the supervisor to plan and prioritize workload. Other duties as assigned.
Qualifications and Abilities:
AA/AS Degree, Bachelors preferred, or a combination of life experience and/or education in
issues related to poverty, families experiencing homelessness, and government benefits.
Demonstrated knowledge of the Metro area resources, government entitlements, and anti-poverty programs for homeless families and Ability to de-escalate and provide support as Experience working with individuals and families from diverse Experience in community-based Training in Trauma Informed Services and Assertive Engagement Possess a conceptual understanding of empowerment, client-centered, family support, and advocacy; possess open communication and positive problem-solving Ability to relate to participants with different learning styles and cultural backgrounds using respectful listening and verbal communication Possess positive problem-solving skills and ability to engage in successful conflict Ability to organize and prioritize multiple Ability to work both in a team-oriented environment and Excellent written communication Proficient skills in Microsoft 365 products - Word, Excel, Teams, and Able to successfully pass a criminal background Valid Oregon drivers license, good driving record, and proof of
