Key Responsibilities:

🔹 Resident Advocacy & Support: Provide emotional and social support to residents and their families, ensuring their rights, needs, and preferences are honored
🔹 Care Planning & Coordination: Collaborate with healthcare professionals, nurses, and therapists to develop comprehensive care plans that enhance residents' quality of life
🔹 Discharge Planning & Community Resources: Assist residents and families with transitions of care, connecting them to community services and ensuring a smooth discharge process
🔹 Crisis Intervention & Conflict Resolution: Address resident and family concerns with empathy and professionalism, working towards positive solutions
🔹 Regulatory Compliance & Documentation: Maintain accurate and up-to-date records, ensuring compliance with state and federal regulations
🔹 Program Development: Implement and oversee social services programs that promote engagement, mental well-being, and an enriched quality of life for our residents

What We're Looking For:

✔️ Bachelor's degree in Social Work OR a bachelor's in a human services field
✔️ At least one year of supervised social work experience in a healthcare setting
✔️ Strong interpersonal skills - ability to connect with residents, families, and team members in a compassionate and professional manner
✔️ A team leader and problem solver who can multitask, adapt to challenges, and advocate for resident well-being
