Assisted Living Nurse Case Manager (Project Management)

Primary Job Purpose: To address the medical, social, psychosocial, and mental health needs of the neighbors and to enhance their quality of life in the Assisted Living.
Customer Service: Adherence to the MOSES program to include Mission, Ownership, Success, Excellence, and Services.
Essential Job Functions
Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
Qualifications, Education, and Experience: Registered Nurse with current license to practice in the state of Colorado. LPN may be considered. Able to supervise QMAP's. Has the patience needed to work with the needs and demands of neighbors. Healthcare experience in a geriatric setting preferred.
Non-Essential Job Functions: May serve on Eben Ezer committees. May need to assist with neighbors' transportation on occasion. Will attend facility meetings and department meetings as instructed.
Other Skills and Abilities:
Material and Equipment used: General office equipment, such as phone, fax, copier, adding machine/calculator, and computer. Equipment associated with patient care such as lifts, tubs, wheelchairs, gait belts, beds, blood pressure cuffs, etc.
Working Environment/Physical Activities: Must be able to sit, stand, and walk 8 hours a day. Must be able to lift and carry 0-50 pounds regularly and 50+ pounds occasionally with assistance. Must be able to push/pull up to 250 pounds regularly using a wheelchair. Sits, stands, bends, walks, reaches, lifts, keyboarding, pushing wheelchairs, and moves intermittently during working hours. Employee is subject to physically ill neighbors as well as hostile and emotionally upset neighbors and/or families.
Working Hours: As scheduled by the Assisted Living Administrator and which may include evenings, nights, weekends and holidays. Occasional shift work on the floor may be required. Schedule may vary, based on needs and supervisor discretion.
Working Location: This is not a remote position and work must be performed in person.
