Outreach and Enrollment Specialist (Patient Education) (Project Management)
The O/E Specialist will receive comprehensive O/E training and apply guidelines to determine patient eligibility for participation in insurance programs.
All SCHC Visit Coordinators will receive basic O/E training.
Provide telephone and in-person assistance to understand and access affordability options and complete insurance and related applications.
Advise consumers of the documentation required to complete applications, and answer questions and issues regarding the enrollment process.
Support social, housing, educational, or other related resources.
Provide information and assistance in a manner that is culturally and linguistically appropriate.
Prepare all related reports and maintains files, data bases and other program records.
Evaluate progress and revise strategies as appropriate.
Work with Front Office Supervisors and Lead Visit Coordinators for new patient assignment to engage potential consumers.
In order to promote maximum enrollment, collaborate with and seek input from Shasta County Health & Human Service Agency (HHSA) "Regional Eligibility Hub" and SCHC HSSA eligibility workers to engage existing and potential consumers.
Work with regional FQHCs and RHCs to build and leverage existing resources and coordinate outreach and enrollment of uninsured individuals and families throughout the county.
Facilitate outreach and education on program services. Coordinate countywide insurance enrollment campaigns to raise awareness.
Serve as a liaison to insurance and other service providers.
Other support duties as approved and assigned.
KNOWLEDGE, SKILLS, AND ABILITIES
Ability to work without direct supervision.
Knowledge of community resources and outreach programs.
Ability to take directions and complete tasks on time.
Demonstrate skill in communicating effectively both orally and in writing.
Knowledgeable and skilled in cultural competency.
Skill in computer programs and applications.
Typing 35 WPM.
EDUCATION & EXPERIENCE
High school diploma or equivalent.
Three years' experience in related field.
Three years' experience in database management and research.
Requires a valid California driver's license.
